Free & Easy to Configure PHP Email Contact Form

I've just found this cool PHP contact form namely Fast & Secure Contact Form by Mike Challis. You can use this whenever you need a multi-fields contact form or any type of forms, such as for your Google Blogger post or page. The form will send you the submitted data to any email you set.

Numerous field types available: from text, textarea to file upload or attachment. It's also secured by CAPTCHA and Akismet.

Email sending is also configurable. Example: to include the name of the form sender, include this tag in the E-mail Subject Prefix: [from_name]. You can put any other label to any part of email.

To display a form on your web page: Just add a few lines of PHP code. Edit the HTML of a PHP page on your web site and add the code provided by clicking 'Show PHP Code' hint on the admin panel.

To use this form, download the zipped script here, extract them and follow the instructions in "readme.txt file". Don't have a web hosting? Don't worry. You can get one free from 000webhost. It supports PHP up to version 5.2.

Need a basic PHP/HTML skill and English reading skill to do this. Just click the blue hint or help on every part to see detailed instructions. See below the full screenshot from its admin panel so you know what I am talking about.
